
When Blood Work Is Normal But You Feel Awful: HTMA Explained | Blue Veil Wellness

As a nurse practitioner with 25+ years in women's health, I hear this story almost every week. A woman comes in with fatigue that sleep does not fix, weight she cannot move no matter what she eats, bloating that will not resolve, and brain fog she has learned to live around. Her labs come back normal. She is offered an antidepressant.
She leaves feeling dismissed. And alone.
This week on the Blue Veil Wellness Show, I sat down with Jennifer Nelson Hawks — a board certified doctor of naturopathy, author of Your Labs Are Lying, and founder of Jennifer Hawks Health. Jennifer experienced this exact scenario herself after the death of her father sent her own health into a spiral that blood work simply could not explain. What she found when she looked deeper changed everything about how she practices.
MEET JENNIFER NELSON HAWKS
Jennifer spent over two decades at the intersection of medicine and holistic health. She owned a wellness practice, sold it in 2022 when burnout hit hard, and then — while navigating grief after losing her father — found herself experiencing everything her clients had described: fatigue that sleep could not touch, weight gain around the midsection, hair loss, bloating, and brain fog.
Her blood work came back normal. She was offered antidepressants and anti-anxiety medication.
She knew better. And she kept looking.
What she found was a functional lab tool called Hair Tissue Mineral Analysis — HTMA — that finally showed her what blood work could not. Her results were the first piece of paper that confirmed something was genuinely depleted. Not in her head. Measurably off at the cellular level.
That moment of seeing proof on paper — after months of being told everything was fine — changed her practice and her life.
WHAT IS HTMA AND WHY DOES IT MATTER?
Hair Tissue Mineral Analysis is a functional lab test that looks at your mineral status at the cellular level — not what is circulating in your blood, but what is actually happening inside each cell over a three-to-four month window.
Here is why that distinction matters.
Standard blood work is designed to detect disease. It reflects what was in your bloodstream at the exact moment the needle went in. What did you eat the night before? Were you dehydrated? Were you stressed that morning? All of that affects the snapshot. And because the body works hard to keep blood mineral levels stable — borrowing from cellular reserves to do it — blood work can appear completely normal even when your cellular reserves are completely gone.
Hair, by contrast, grows slowly. The first inch of hair reflects a three-to-four month window of what your body has actually been doing at the cellular level. That is a fundamentally different kind of information.
The process is simpler than you might expect: a kit arrives at your home, you wash your hair with clarifying shampoo, rinse with distilled water, air dry, and clip a small amount (about a teaspoon) from the back of the head. Results come back in seven to ten days.
What HTMA reveals:
- Your oxidation rate — how fast or slow your metabolism is actually running
- Your adrenal resilience — whether your stress response is over-firing or completely exhausted
- Your thyroid pattern at the cellular level
- Your mineral ratios and how they are communicating with each other
- Your body's current capacity to detoxify
- Whether toxic metals are present and whether the body has the energy to move them
THE MOST COMMON BURNED-OUT PATTERN JENNIFER SEES IN WOMEN
The pattern Jennifer described stopped me mid-conversation.
High intracellular calcium and magnesium that are completely bio-unavailable to the body — meaning they are present, but the body cannot use them. Combined with rock-bottom sodium and potassium — the minerals that drive energy production, digestion, adrenal function, and the stress response.
When sodium and potassium are depleted at the cellular level, the adrenals lose the capacity to do their job. The thyroid slows. Digestion stalls. Weight accumulates. Fatigue sets in that no amount of sleep addresses.
And standard blood work says everything is fine.
Jennifer's second pattern: "four lows." All four primary minerals critically depleted. This is a body that is holding onto everything — sequestering toxins it cannot release, draining the drainage pathways, and becoming hypersensitive. Women in this pattern are sensitive to smells, foods, people, environments. They feel everything, and not in a good way. These women need to go low and slow. Pushing harder makes things worse.
THE CALCIUM SUPPLEMENT PROBLEM
This is the part I found most clinically significant.
Many women with bone density concerns are placed on high-dose calcium supplements. But HTMA frequently reveals that these women have HIGH intracellular calcium — calcium that has been pulled from the bones by chronic stress and is now stuck in soft tissues, bio-unavailable and unusable.
Adding more calcium does not fix it. The body needs help shuttling the calcium it already has back to where it belongs.
Meanwhile, blood work and DEXA scans both reflect low calcium — so the prescription continues. HTMA tells an entirely different story.
WHY MINERALS WORK IN RATIOS, NOT IN ISOLATION
One of the most important things Jennifer said was this: minerals do not work alone.
Sodium and potassium must be in the right ratio. Calcium and magnesium must be in the right ratio. Zinc and copper must be in balance. The communication between these minerals is what drives metabolic function, hormonal production, immune resilience, and digestive capacity.
This is also why self-prescribing magnesium — because social media told you everyone needs it — may do nothing, or even make things worse. If your calcium is high and bio-unavailable, magnesium follows it and becomes bio-unavailable too. You can take the right supplement and get no result because your specific mineral pattern cannot use it the way you expect.
The same applies to electrolyte drinks. The ratio of sodium to potassium in those products may be the opposite of what your particular body needs. Without knowing your HTMA pattern, you are guessing.
WHEN TO DETOX — AND WHEN NOT TO
Many women want to jump straight to detoxing when they start feeling better. Jennifer is direct about this: detox is a second or third stage, not a first stage.
On the first HTMA test, toxic metals often do not even appear — because the body does not have enough cellular energy to mobilize them. They are sitting quietly and will not show until the mineral foundation is strong enough for the body to start moving them.
Rushing into detox protocols before the foundation is solid typically leaves women feeling worse, not better. Build the reserves first. Then the body can move what needs to move, with support.
A CLINICAL NOTE FROM CRISTY
This conversation confirmed something I see regularly in my perimenopause practice: the body is always communicating. The weight that will not move, the fatigue that will not lift, the hormones that will not balance — often there is a depleted mineral foundation underneath that no amount of hormone adjustment will fix.
I recently had a patient presenting with classic perimenopause symptoms who was also training for a half marathon and managing a stressful home situation. There was no dose of estrogen that was going to be the answer until the adrenal reserves and mineral foundation were addressed first.
HTMA is the kind of testing that helps us see that picture. And for the women who have been told their labs are normal while feeling anything but — this may be exactly the piece they have been missing.
